GraceAChild Enrollment Process

 

The following information are some key points to remember when completing your enrollment form. For questions when enrolling, please call: (540) 382-9591 and ask for Lesley Lively, Director.

PARENT/GUARDIAN INFORMATION:
First parent/guardian listed is considered “Primary Payer”. Second parent/guardian is also required unless legal documents are presented stating otherwise.

REQUIRED EMAIL ADDRESS:
We prefer you provide a personal email address (that you can readily check as needed) as business emails often see our messages as Spam, and can usually be viewed by third parties. If your preference is to use your business email, please be sure to save our address in your contacts and ensure that our emails cannot be viewed by others to help protect your private information.

EMERGENCY CONTACTS INFORMATION:
Emergency contacts are family, friend, co-worker, neighbor, etc., who live within one hour of GraceAChild Center. Phone number & full physical addresses are required for contacts per state licensing guidelines. 

UPDATING CONTACT INFORMATION:
Keeping your information updated while your child is enrolled is very important, especially in the case of an emergency. Please contact your child’s center director immediately if you have any changes in either parent/guardian’s phone number, address, email, phone provider, emergency contacts and their perspective contact information.
